This website is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use the care guide, the portal, or email to seek emergency help.
1. Agreement
These terms govern your use of the Aponi Health website and patient portal, operated by Aponi Health in San Francisco, California. By creating an account or booking a visit you agree to them. If you do not agree, please do not use the site.
2. What this website is, and is not
- The website lets you learn about our services, book appointments, complete intake paperwork, and view your visit history. Care itself happens in person at the clinic with licensed practitioners.
- The AI care guide is educational. It helps you decide which of our services may fit your situation. It is not medical advice, does not diagnose or treat any condition, and does not create a practitioner-patient relationship. A relationship with a practitioner begins when they actually evaluate you.
- Information on this site is general and is no substitute for the judgment of a licensed professional who has examined you.
3. Your account
- You must provide accurate information and keep it current; your care team relies on it.
- Keep your password private. You are responsible for activity on your account, and you should tell us immediately if you suspect someone else has accessed it.
- Accounts are for people 18 and older. Care for minors is arranged directly with the clinic by a parent or guardian.
4. Appointments and cancellations
- Booking a slot reserves your practitioner's time. If you cannot make it, please cancel or reschedule in the portal.
- We ask for at least 24 hours notice. Late cancellations and no-shows may be subject to a fee under the clinic's posted policy at the time of booking.
- We may need to reschedule for practitioner illness or clinic closures; we will tell you as early as we can.
5. Acceptable use
Do not misuse the site: no attempts to access other people's records, probe or overload our systems, submit false information, or use the site for anything unlawful. We may suspend accounts that put other patients' data or the clinic's systems at risk.
6. Communications
With your consent we send transactional email: booking confirmations, appointment reminders, and account messages such as password resets. You can turn reminders off in the portal. Email is not monitored around the clock and is not a way to reach your practitioner urgently.
7. Privacy
Your privacy is governed by our Privacy Policy and, for medical information, our Notice of Privacy Practices. Both are part of these terms.
8. Intellectual property
The site's content, design, and branding belong to Aponi Health. You may use the site for your own care; you may not copy or reuse its content commercially without permission. Your medical records are yours to access and export at any time.
9. Disclaimers and limits
- The website is provided "as is." We work hard to keep it available and accurate but cannot promise it will always be error-free or uninterrupted.
- To the maximum extent permitted by law, Aponi Health is not liable for indirect or consequential damages arising from use of the website itself, such as scheduling errors caused by outages.
- Nothing in these terms limits or waives any right you have under law regarding the professional care you receive, and nothing here alters the professional obligations of our licensed practitioners.
10. Governing law
These terms are governed by the laws of the State of California. Any dispute about the website will be handled in the state or federal courts of San Francisco County, California.
11. Changes
We may update these terms. The current version lives at this page with its effective date; significant changes will be announced in the portal or by email, and continued use after that means you accept them.
